• Provide comprehensive and person-centred assessments for adults in the community, ensuring their needs are met and independence is promoted.
• Collaborate with a multi-disciplinary team to create and implement care plans tailored to the unique needs of each individual.
• Advocate for service users, supporting them through the complexities of social services, and liaising with relevant agencies when necessary.
• Make informed decisions and manage risk effectively while maintaining a high standard of professional practice.
• Regularly review and update care plans, ensuring the well-being and safety of service users are at the forefront of your work.

• Degree in social work or an equivalent qualification and current registration with Social Work England.
• Significant frontline experience in UK Social Services as a qualified Social Worker, demonstrating a strong track record in community settings.
• Excellent analytical and communication skills, enabling you to navigate challenging situations with calm and efficiency.
