Health Care Assistant

This is an exciting opportunity to join our evolving Integrated Neighbourhood Team across Bosvena and Three Harbours as aHealthcare Assistant.

Working under the supervision of the Lead Nurse, the Healthcare Assistant will support the delivery of safe, proactive and patient-centred care across the Integrated Neighbourhood Team.

The post holder will undertake a range of delegated clinical duties including health checks, phlebotomy, ECG recording,wound care support, learning disability health checks, chaperoning, assisting with procedures, processing laboratory samples and supporting clinical stock,vaccine and equipment management.

The role will help patients access timelycare and support the wider team to deliver personalised, preventative andcoordinated long term conditions care across our local population.

Clinical care and patient support

Undertake delegated duties in accordance with agreed protocols, training and competencies

Support the delivery of health checks, including long term conditions checks, new patient checks and learning disability health checks

Undertake phlebotomy, ECG recording and physical health observations

Support wound care and ulcer care

Act as a chaperone and assist patients where appropriate during clinical examinations or procedures

Assist clinicians with minor surgery and other clinical procedures where required

Proactive and Integrated Neighbourhood Team working

Work alongside practice teams and wider health and care partners to support patient-centred care

Support proactive care for people with frailty and long-term conditions

Help patients access timely care, follow-up and support

Contribute to the identification of patients who may benefit from additional support, health checks, monitoring or follow-up

Promote health and wellbeing, prevention and personalised care

Clinical administration, safety and quality

Process and manage laboratory samples

Maintain accurate patient records and clinical coding

Prepare and maintain clinical environments and equipment

Sterilise, cleanse and maintain surgical equipment

Support vaccine/cold chain storage, monitoring and recording

Support clinical stock control

Clear and re-stock consulting and treatment rooms

Apply relevant standards and guidance

We aredeveloping our Integrated Neighbourhood Team, working with general practice,social care, community partners, secondary care and voluntary sector providers to deliver high-quality, patient-centred health care across our geography.

Our focus is on providing more proactive,preventative and coordinated care for local people, including those living withfrailty, long-term conditions and complex needs. The Healthcare Assistant willplay an important role in supporting this work by helping patients accesstimely care, health checks, monitoring and follow-up as part of the wider

Integrated Neighbourhood Team.

Job responsibilities

Clinical care and patient support

The post holder will undertake delegated clinical duties in accordance with agreed practice policies, protocols, training and competencies. Duties may include:

New patient health checks

Long terms conditions checks, Learning disability health checks, including physical health observations, weight, heart rate, blood pressure, screening bloods where appropriate, vaccination review and discussion of health and wellbeing with patients and/or carers

ECG recording

Physical health observations

Wound care and ulcer care support, where trained and competent

Support for immunisation programmes, where trained and competent

Chaperoning and assisting patients where appropriate during examination or treatment by another clinician

Assisting GPs, nurses and other clinicians with minor surgery or other clinical procedures where required

Undertaking specific clinical activities for named patients where these have been delegated, taught and assessed as competent

Proactive and Integrated Neighbourhood Team working

Work alongside GPs, nurses, care coordinators, community nursing teams and wider health and care partners to support coordinated, patient-centred care

Support proactive care for people with frailty and long-term conditions

Help patients access timely care, follow-up and appropriate support from the wider Integrated Neighbourhood Team

Contribute to the identification of patients who may benefit from additional support, health checks, monitoring or follow-up

Assist in the assessment and surveillance of patients health and wellbeing

Help raise awareness of health and wellbeing, prevention and personalised care

Assist with the collection and collation of data relating to health and wellbeing needs

Clinical administration, equipment and safety

Process and manage laboratory samples requested by GPs, nurses and other clinicians

Maintain accurate patient records and clinical coding relevant to the role
