We are a friendly and supportive team of people with diverse backgrounds, highly motivated to drive change and improve the lives of disabled people, their carers and families. We take a flexible approach to supporting each member of our team to reach their full potential.

As a Personalised Support Specialist within the Humans Cornwall team, you will provide personalised, practical and strengths-based support to disabled people, people living with long-term health conditions and their carers across Cornwall and the Isles of Scilly.

Working across different service pathways, you will help people address immediate challenges, build confidence and maintain or improve their independence, wellbeing and resilience at home and within their community.

Depending on the person’s circumstances, this could include supporting someone to return home safely following a hospital stay, helping to prevent an avoidable admission, providing short-term practical support.
This role does not provide personal care.

You will work with a range of colleagues, professionals, delivery partners and community organisations to deliver the support agreed with each person. Every situation will be different, requiring you to be compassionate, resourceful and responsive while respecting each person’s choices, strengths and expertise in their own life.

  • Occasionally provide practical support, such as shopping, domestics and life administration.
  • Build positive and respectful relationships with clients.
  • Work collaboratively with internal teams to ensure agreed support is delivered effectively.
  • Liaise appropriately with health and social care professionals, voluntary and community organisations and other stakeholders involved in the person’s support.
  • Help people access local services, community resources, peer support, and opportunities that can continue beyond the period of direct support.
