Position Overview
The Registered Practical Nurse (RPN) – Wellness & Foot Care is responsible for the provision of wellness support and management of cost‑effective foot care services in Community Centers located in Lancaster, Alexandria, Finch, Ingleside and Winchester or the client’s home. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the provincial regulatory body (Ontario College of Nurses).

The RPN will utilize a General and Diabetic Foot Assessment Tool which follows the Registered Nurses Association of Ontario Best Practice Guideline.

Responsibilities

Provide wellness support and foot care services to clients in community centers and residential settings.

Perform general and diabetic foot assessments using the designated assessment tool.

Adhere to quality and risk management frameworks, provincial regulations, and Carefor standards.

Document care activities, assessments, and outcomes in patient records.

Collaborate with the interdisciplinary team to coordinate comprehensive care plans.

Qualifications

Registered Practical Nurse (RPN) in Ontario.

No employment in Ontario during the six months preceding hiring.

Knowledge of and ability to use the General and Diabetic Foot Assessment Tool following best practice guidelines.

Strong communication and interpersonal skills.
