We are seeking a dedicated and compassionate Social Worker to join our Residential Social Services team. The successful candidate will play an significant role in supporting the emotional, psychological and social wellbeing of residents and where appropriate, their families. The role involves close collaboration with a multidisciplinary team to ensure the highest standard of holistic care.
Role and Responsibilities- Conduct psychosocial assessments and provide ongoing casework and support to residents
- Develop, implement and review individual care, support and intervention plans
- Facilitate family communication, meetings and case discussions
- Provide counselling and crisis intervention when required
- Coordinate referrals to specialised services
- Facilitate therapeutic and support groups
- Maintain accurate records, reports and statistics
- Participate in multidisciplinary team meetings and care planning
- Liaise with internal departments and external stakeholders
- Engage with external regulatory bodies such as the Department of Social Development (DSD) and Department of Health (DOH) where required
- Ensure compliance with relevant legislation, organisational policies and skilled ethical standards
- Bachelor’s degree in Social Work
- Current registration with SACSSP
- 2–3 years’ relevant experience
- Solid communication and report‑writing skills
- Ability to work effectively within a multidisciplinary team
- Empathy, compassion and professional integrity
- Solid interpersonal and communication skills
- Ability to manage challenging situations calmly and effectively
- Problem‑solving, decision‑making and conflict‑resolution skills
- Ability to work independently and under pressure
- Good organisational and time‑management skills
- Computer literacy (MS Office 365: Word, Excel, PowerPoint)
- Counselling and facilitation skills
- Experience working with elderly persons and/or individuals with cognitive decline
- Experience in behavioural management and care planning
- Experience in the mental health and/or disability sector
- Valid driver’s licence
