Key Responsibilities:
• Complete detailed patient assessments incorporating clinical and psychosocial needs

• Coordinate access to health and community care services
• Communicate effectively with the multidisciplinary healthcare team
• Document patient interactions per Ontario Health atHome standards
Requirements:
• Registered Nurse, Physiotherapist, or equivalent relevant degree
• Minimum two years of related professional experience
• Bilingual in English and French
• Valid driver’s license and access to a vehicle
• Vulnerable Sector Check completed within six months
