Responsibilities
ADMIN ASSISTANT – CASE MANAGEMENT
Full‑time, days – Corona Regional Medical Center
Reports to the Director of Case Management. Works under the direct supervision of the department manager, unit case manager or social worker. Provides support and works collaboratively with the multidisciplinary healthcare team, patient/family, physicians and community resources to anticipate and plan for discharge needs. Assists with coordinating all components of the discharge plan to facilitate a safe transition from acute care to the post‑acute care setting.
Duties include meeting with families to discuss the discharge plan, seeking assistance for low‑income or indigent patients, communication with insurance companies, faxing, copying, accurate and timely documentation in MIDAS. Proactive in assuring delivery of an appropriate, effective, high quality and safe discharge in a cost‑effective manner. Facilitates system/process improvements to reduce/remove barriers to discharge. Participates and promotes the processes and projects of the Case Management Department.
Provides education to the healthcare team regarding the Case Management program functions, goals, and outcomes as needed.

- Some college education preferred
- Two years experience in the health care field preferred
- Organized with excellent verbal and written communication skills and computer literacy
- Familiarity with community resources, medical terminology, and computer literacy
- Knowledge of insurance practices, Medicare, Medi‑Cal
- Able to work comfortably with physicians, nurses, clinical staff, patients, families, insurance and community agency representatives
