Position: Clinical Liaison (RN ONLY-HOSPICE)

Overview

Altus Hospice is looking for a self‑motivated and passionate Hospice RN Clinical Liaison to join our team. The RN Clinical Liaison will conduct hospital and medical facility outreach to identify potential referral sources, maintain ongoing relationships, and increase hospice census and preference.

Monday – Friday, 8:00 am – 4:30 pm

Responsibilities
  • Identify primary decision makers and proactively build relationships with medical community members and partners.
  • Promote Hospice patient care services through regular visits that reflect Hospice quality and capabilities.
  • Build Hospice brand as the preferred provider to achieve and exceed agency growth and census goals.
  • Make scheduled calls to secure new referrals, build census, and meet goals with physicians, facilities, case managers, discharge planners, social workers, and other providers.
  • Assist Hospice in determining patient eligibility based on hospice knowledge and training.
  • Conduct initial patient and family meeting to overview Hospice services, philosophy, and obtain consent for care.
Qualifications
  • Bachelor’s degree in Nursing or Health Care (preferred).
  • Current registered nurse license, unencumbered.
  • Minimum one year of sales/marketing experience in healthcare.
  • One to two years of hospice/palliative marketing experience strongly preferred.
  • Skilled at establishing and maintaining working relationships with key staff, marketing contracts, and clinical staff to ensure thorough understanding of Hospice services.
  • Proven ability to generate and maintain relationships with new and existing accounts including physicians, health professionals, and community agencies.
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.
Salary

$65,000 – $80,000 per year (Average Pay Range). This is a general guideline; final compensation will be based on experience and responsibilities and confirmed at the end of the interview process.
