• Provide nursing care in the home: wound care, medication administration, support during procedures.
• Document assessments, interventions, outcomes clearly.
• Monitor patient status, alert care team of changes.
• Educate patients and families on medication adherence and health conditions.
• Collaborate closely with RNs, providers, therapists, coordinators.

• LPN/LVN license.
• Basic Cardiac Life Support within 30 days of start.
• Driver license.
• Vocational school graduate.
• 1 year experience required.
