Base pay in range will be determined by applicant’s skills and experience. Role is also eligible for team based bonus opportunities. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.

- Conduct and coordinate the development and completion of resident assessments in accordance with State, Federal, and company guidelines.
- Coordinate the generation of the Minimum Data Set (MDS) for each Medicare patient and ensure electronic transmission within the mandated timeframe.
- Coordinate interview(s) of each resident; evaluate resident’s condition and medical data; implement procedures with the Director, Clinical Services for newly admitted residents.
- Ensure all assessments are completed and transmitted on time; assist community directors and supervisors in scheduling resident assessment/care plan meetings.
- Assist in scheduling participation by outside members of the care plan team, including the resident’s representative and/or family members.
- Develop written plans of care, including measurable objectives and timetables.
- Maintain and periodically update policies and procedures governing resident assessment and care plans; ensure availability of the MDS Instructor’s Manual.
- Implement and maintain an ongoing quality assurance program; monitor QI and QM reports and take corrective action when necessary.
- Assist the Discharge Planning Coordinator with the care plan portion of the resident’s discharge plan and participate in discharge planning functions as needed.
- Participate in community surveys (inspections) by authorized government agencies.
- Provide leadership and participate in committees such as the Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
- Develop and facilitate in‑service training classes on assessment skills and techniques needed for MDS functions; assist the In‑service Director/Educator; attend annual training programs.
- Nursing Degree from an accredited college or university.
- Minimum two (2) years of experience as a supervisor in a hospital, nursing care community, or related facility.
- Minimum six (6) months training experience in rehabilitative and restorative nursing practices.
