Delivers evidence-based nursing care and patient education related to diabetes management, healthy lifestyle practices, and obesity prevention to improve health outcomes and reduce barriers to care. Serves as a clinical resource and educator by conducting community wellness classes, providing patient education, promoting disease prevention, and collaborating with healthcare providers and community partners to improve population health throughout the service area.

* ESSENTIAL DUTIES AND RESPONSIBILITIES:

*** Provide nursing services, health education, preventive care, screenings, and care coordination support to individuals across the lifespan in accordance with professional standards and organizational policies.
* Conduct low- or no-cost chronic disease screenings in accordance with established clinical protocols, with an emphasis on prevention, early detection, patient education, and appropriate referral for follow-up care.
* Assess, treat, and manage acute and chronic health conditions within the RN scope of practice.
* Develop individualized care plans that address physical, behavioral, nutritional, and lifestyle factors affecting health outcomes.
* Deliver evidence-based diabetes management services, including assessment, monitoring, patient education, and care coordination.
* Implement evidence-based obesity-related interventions designed to improve health outcomes and support healthy lifestyle changes.
* Provide individualized patient education regarding disease prevention, medication management, nutrition, physical activity, and self-management of chronic conditions.
* Host and facilitate community health and wellness classes focused on disease prevention, diabetes management, obesity reduction, healthy living, and other community health priorities.
* Collaborate with the Registered Dietitian to provide coordinated nutrition and wellness services.
* Promote preventive healthcare services through community outreach, screenings, health fairs, and educational events.
* Identify patient risk factors and make referrals to appropriate medical, behavioral health, social service, and community resources.
* Review, monitor, and communicate diagnostic test results to the appropriate healthcare provider and coordinate follow-up care in accordance with organizational policies and scope of practice.
* Administer medications and treatments as ordered by an authorized healthcare provider and in accordance with state regulations, organizational policies, and nursing standards.
* Maintain accurate, timely, and complete documentation within the electronic health record and other required systems.
* Monitor patient progress, evaluate responses to interventions, and update nursing care plans as appropriate in accordance with scope of practice and organizational policies.
* Collaborate with physicians, nurses, dietitians, therapists, community organizations, and other healthcare professionals to ensure continuity of care.
* Assist with collection, tracking, and reporting of program data, outcomes, performance measures, and quality indicators.
* Participate in quality improvement initiatives designed to improve patient outcomes and enhance program effectiveness.
* Provide care appropriate for the patients served based on demonstrated knowledge of the principles of growth and development over the life span.
