Job Title:

Patient Care Coordinator and UA Collector

Porch Light Health is a respected leader in the outpatient treatment of Opioid Use Disorder (OUD) and Alcohol Use Disorder (AUD) and other addictive disorders. This role is embedded in The Recovery Center (TRC) which provides court ordered care in Cortez, CO and surrounding communities. We foster a supportive and family‑oriented environment where patients are treated with compassion, respect, and honesty.

Our team believes that addiction is a chronic, relapsing brain disease, and we are passionate about helping individuals navigate their recovery journey. We provide behavioral health services, Medication‑Assisted Treatment (MAT), and psychiatric care helping patients discover their purpose and build a healthy community. We are seeking a dedicated and compassionate Patient Care Coordinator (PCC) and UA Collector to join our growing team in a fast‑paced, dynamic medical setting.

This entry‑level position offers comprehensive training and the opportunity to make a direct impact on the lives of patients.

Responsibilities and Duties
  • Collecting Urine Drug Screens:
    This role primarily collects observed urine samples in our lab and documents in our medical record system.
  • Patient Scheduling:
    Coordinate patient appointments and ensure timely and accurate scheduling.
  • Collecting Vitals & Patient Information:
    Take and record vital signs and gather essential patient information.
  • Phone Handling & Patient Interaction:
    Answer phone calls, assist patients with inquiries, and provide direction as needed.
  • Patient Registration:
    Admit and register patients into the clinic system, ensuring all necessary documentation is completed.
  • Data & Record Keeping:
    Accurately input patient data into the clinic’s electronic health record (EHR) system and maintain updated patient files.
  • Facility Maintenance:
    Assist with building upkeep, cleaning, and ensuring the clinic environment is safe and welcoming.
  • Communication with Medical Staff:
    Relay information between patients and medical providers, ensuring smooth communication and efficient care.
  • Customer Service & Patient-Centered Care:
    Provide exceptional service to patients by assisting with inquiries, directing them to appropriate services, and fostering a compassionate and supportive clinic atmosphere.
  • Crisis Management & De-escalation:
    Demonstrate competency in verbal de‑escalation, crisis intervention techniques, and maintaining a calm, professional demeanor in stressful situations.
  • Regulatory Compliance:
    Follow HIPAA, OSHA, and other safety protocols to ensure patient confidentiality and a safe work environment.

Qualifications and Skills
  • Previous

    Experience:

    Experience in a medical clinic or addiction treatment setting is preferred. Willingness to learn and grow in the field is equally valued.
  • Education:

    High school diploma or GED equivalent required. Experience can substitute for educational requirements in some cases.
  • Technical

    Skills:

    Basic computer skills, including proficiency with office software and EHR systems.
  • Transportation:
    Reliable transportation is required for commuting to the clinic.
  • Time Management:
    Strong organizational and time management skills are essential.
  • Dependability:
    Must be reliable, punctual, and able to work effectively in a team environment.
  • Licenses/Certifications:

    Valid driver’s license required.
Job Type
  • Part‑Time
  • This position may require flexibility with hours, including 3‑6 hour shifts (4‑7pm Monday thru Friday and 1:30‑7pm on Saturday or Sunday). Due to requirements for collecting random drug screens, we are required to be open one weekend day from 12‑5pm.
