Compensation: $40.00 - $53.00 per hour
Summary:

The Annual Wellness Visit (AWV) Nurse works collaboratively with the primary care provider, care coordination team, and all members of the healthcare team. The AWV Nurse identifies Medicare patients who are eligible for an AWV and executes a guided process to complete the AWV under the direct supervision of the patient’s primary care provider. This position requires a candidate who can effectively work with a diverse group of patients and stakeholders within the healthcare setting.

Principal Responsibilities:
  • Participate and maintain a culture within The Corvallis Clinic consistent with the content outlined in the Service and Behavioral Standards Handbook.
  • Engage and empower patients to participate in the preventative care approach.
  • Identify when patients are eligible for AWV that are on the schedule and AWV gap lists.
  • Work collaboratively with clinical teams to accomplish AWV efficiently for the patient and care team.
  • Follow AWV protocols and standing orders, including completion of preventative services that may result from AWV.
  • Develop and share knowledge of AWV eligibility and documentation requirements.
  • Address open quality gaps with patients and work collaboratively with them toward education and closure of these gaps.
  • Document appropriately within the electronic medical record (EMR).
  • Utilize appropriate protocols and standing orders in delivering patient care.
  • Participate in regular team meetings, huddles, staff meetings, and quality improvement projects to improve patient care delivery.
  • Consult with clinical staff and ancillary department staff to reduce barriers to the efficient delivery of care.
  • Maintain clinical competencies for the role.
  • Assist with other clinical duties in the Quality Department and Care Coordination team, as needed.
  • Education, Licensure and

    Experience:

    • Current Oregon Licensed Practical Nurse or Registered Nurse required.
    • BLS Certification required within 90 days of hire.
    • Preferred experience in primary care, community nursing, critical care, or home health.
    Knowledge and

    Skills:

    • Ability to work independently and exercise clinical judgment in interactions with providers, patients, and their support person(s).
    • Strong organizational and time management skills.
    • Awareness of scope of practice boundaries and comfortable seeking direction and assistance.
    • Demonstrated initiative and good professional judgment.
    • Proficient user of Microsoft programs and Electronic Medical Records.
    • Maintain confidentiality and comply with all HIPAA regulations.
    • Understanding and appreciation of Social Determinants of Health (SDoH) and Preventative Medicine.
    • Strong team player with passion for interdepartmental collaboration.
    • Display a positive attitude and act as a positive agent for change.
