Summary:
The Annual Wellness Visit (AWV) Nurse works collaboratively with the primary care provider, care coordination team, and all members of the healthcare team. The AWV Nurse identifies Medicare patients who are eligible for an AWV and executes a guided process to complete the AWV under the direct supervision of the patient’s primary care provider. This position requires a candidate who can effectively work with a diverse group of patients and stakeholders within the healthcare setting.
Principal Responsibilities:Experience:
- Current Oregon Licensed Practical Nurse or Registered Nurse required.
- BLS Certification required within 90 days of hire.
- Preferred experience in primary care, community nursing, critical care, or home health.
Skills:
- Ability to work independently and exercise clinical judgment in interactions with providers, patients, and their support person(s).
- Strong organizational and time management skills.
- Awareness of scope of practice boundaries and comfortable seeking direction and assistance.
- Demonstrated initiative and good professional judgment.
- Proficient user of Microsoft programs and Electronic Medical Records.
- Maintain confidentiality and comply with all HIPAA regulations.
- Understanding and appreciation of Social Determinants of Health (SDoH) and Preventative Medicine.
- Strong team player with passion for interdepartmental collaboration.
- Display a positive attitude and act as a positive agent for change.
