GENERAL DESCRIPTION

The Rapid Rehousing Case Manager is a grant-funded position focused on providing case management and rental assistance to Veteran households. The person in this role will provide direct services to individuals, which include eligibility screening, program enrollment, participant follow-ups and check-ins. Case managers will also be responsible for processing required paperwork, data entry and other general program supports. This position will directly align with the Housing First initiatives.

This is a grant-funded position contingent upon the continuation of program funding.

MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required.

EDUCATION and/or EXPERIENCE
  • High School Diploma or GED.
  • Four years of relevant social services experience.
  • Knowledge of/experience with the principles of trauma-informed care, trauma awareness, working with veterans' population, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery is preferred.
  • Equivalent combination of education and experience may be accepted.
  • Veterans' status preferred (not required).

CERTIFICATES, LICENSES, REGISTRATION

  • Driver qualifications are required for this position. Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
  • Basic Life Support/First Aid Certification is required within first 30 days of hire.
  • Food Handler's Card or ability to obtain within 30 days.
  • Candidate must pass pre-employment and random drug screenings.
  • Candidate must pass Oregon Department of Human Services background check prior to employment.
  • Candidate must pass comprehensive MWVCAA background screening prior to employment.

KNOWLEDGE, SKILLS, & ABILITIES

  • Must possess computer skills including Microsoft Outlook, Word, and Excel, data entry, and internet applications;
  • Effective customer service, phone skills, as well as writing and appropriate email etiquette.
  • Must possess time management skills, meet deadlines, and process cases accurately with a sense of urgency.
  • Exhibits a positive attitude and engages with others in a cooperative and supportive manner.
  • Must be willing to work day, evenings and weekend shifts as needed.

ESSENTIAL DUTIES and RESPONSIBILITIES This job description is not intended to be all-inclusive. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.

  • Facilitates the distribution of rapid re-housing and barrier removal funds. Ensures clients have access to housing assessments and resources.
  • Assesses client eligibility and aids in gathering documentation as required for intake, enrollment, and financial assistance.
  • Provides direct service to participants, including case management and housing placement support to assist participants in becoming self-sufficient and overcoming barriers to accessing permanent housing.
  • Develops Individual Service Plans that identify participant needs, barriers, and strengths.
  • Establishes and maintains an active working relationship with community partners.
  • Provides support for client-tracking, service delivery, follow-up reporting, maintaining strict confidentiality.
  • Attends and engages in required annual trainings.
  • Maintains full compliance with all federal, state, and grant program/funding requirements, remaining in compliance with the regulations of all funding streams.
  • Maintain high standards for data quality and compliance performance.
  • Works cooperatively with other agency programming to ensure quality and timely client service delivery.
  • Complies with agency quarterly and annual reports.
  • Complies with all Agency policies, including finance, administration, and safety and health policies.
