Clinical Pharmacist - Clinic
Hoag Health System is seeking a licensed pharmacist to apply knowledge, skill, competence, and clinical expertise in the outpatient Hoag Clinic.
The Clinical Pharmacist - Clinic will work with a multidisciplinary team, primarily in the Hoag Clinic as a direct patient care clinician. Responsibilities include working closely with providers to manage medications, ordering and interpreting laboratory assessments, and contributing to training and education of other clinicians.
The pharmacist will provide medication therapy management, including examining patients’ medical records, performing comprehensive medication reviews, and analyzing medication lists for potential patient safety problems.
Additional responsibilities include co-managing high-risk patients with PCPs through in-depth disease education, home blood pressure monitoring review, diabetes coaching, and addressing medication barriers.
Qualifications
- Graduated from an ACPE-accredited pharmacy school program
- At least 3+ years experience in ambulatory care setting or related services
- Experience with value-based programs, including Medication Adherence and HEDIS/PQA metrics
- Valid license to practice pharmacy in the State of California
- Board of Pharmacy Specialties certification preferred
- Current BLS and ACLS certification
