Physical Therapist (PT) - Post Acute Care

Location:

Cottage Grove Post cute

Key Responsibilities Patient Evaluations:
Conduct comprehensive initial assessments, taking into account medical history, functional limitations, and patient goals.

Treatment Planning:
Develop and execute customized, evidence-based physical therapy programs.

Hands-on Care:
Utilize manual therapy techniques, therapeutic exercises, and modalities to facilitate recovery.

Documentation:
Maintain accurate, timely electronic medical records (EMR) in compliance with regulatory standards.

Collaboration:

Communicate effectively with physicians, family members, and other healthcare team members to coordinate optimal patient care.

Education:

Instruct patients and caregivers on home exercise programs and injury prevention strategies.

Qualifications & Requirements

Education:

Graduate of an APTA-accredited physical therapy program (Master’s or Doctorate degree).Licensure:
Current or eligible State Physical Therapist License.

Certification:
Current Basic Life Support (BLS) / CPR certification- preferred but not required

Skills:

Strong clinical reasoning, excellent interpersonal communication, and a team-first mindset. New graduates are welcome to apply and will be paired with a structured clinical mentor.
