Summary of WorkAssesses and monitors the client’s ongoing psychiatric, medical and social needs. Oversees medication monitoring and administration under team psychiatrist/nurse practitioner. Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment. Manages pharmaceutical and medical supplies.
This position functions as a fully integrated Team Member of the Assertive Community Treatment Team that assists individuals with chronic mental illness to minimize symptoms and the resulting impairment in daily functioning.
Compensation: $53,100 - $67,177 per year (salary determined by total years of relevant experience)
Work Schedule: 40 hours per week, Monday through Friday, 8:30 a.m. to 5:00 p.m.
- Must be a graduate of an accredited school of nursing and licensed in the state of Iowa as an RN.
- Two years of experience, preferably in a substance abuse or mental health behavioral health setting along with working knowledge of primary health care issues.
- Plan, implement and evaluate programs of care and therapeutic intervention to meet the needs of an identified caseload of chronically mentally ill individuals.
- Act as case manager both to clients on own caseload and that of other professionals on the team.
- Ensure that care given is appropriate and coordinated, incorporating best practice for treatment, risk assessment and multi‑agency service coordination.
- Support individuals and their families during inpatient admission and with their reintegration to the community.
- Provide psychiatric assessments for individuals to include routine and emergency assessments.
- Assess physical health needs, make appropriate referrals, and coordinate care as needed.
- Screen for health risks at time of admission and periodically thereafter.
- Connect clients who have no primary care providers with a provider.
- Health Coaching – implement individual and group wellness activities.
- Provide crisis health intervention to stabilize health of ACT clients when at risk for admission or readmission or ER use.
- Implement preventive, as well as remedial, health and wellness plans.
- Cross‑train other team members in use of health and wellness interventions as individually indicated.
- Provide management and administration of medication in conjunction with the ACT psychiatrist or nurse practitioner.
- Work closely with other members of the team and staff from other key agencies.
- Provide a range of therapeutic interventions to individuals and their families.
- Maintain client clinical record with treatment plan, client progress and termination summary.
- Provide education and training to agency staff regarding health issues.
- Display a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
- Cooperate and collaborate with program area staff, volunteers, and other Agency staff.
