Care Coordinator Pay Range

The Care Coordinator identifies patients with preventative and chronic condition care gaps and performs patient outreach to close gaps.
Schedule & PayThis is a full‑time (40‑hour) position, Monday‑Friday (8 am‑5 pm). Pay Range: $19–$21/hour, based on experience and education level.
Duties- Schedules and tracks referrals for specialty care.
- Schedules patients for specialty referrals and diagnostic studies.
- Processes incoming and outgoing referrals through the Unite Us portal within 48 hours.
- Requests records from specialists within 30 days after referral appointments have been completed.
- Works with patients to identify barriers to care and helps identify resources to address barriers (e.g., transportation, etc.).
- Communicates information appropriately with care teams related to patients’ needs.
- Tracks referrals through to completion and documents status in EHR.
- Works with supervisor to set weekly/monthly referral goals and collects data to monitor progress of established goals.
- Participates in quality‑related work groups, as assigned. Actively participates in preventative care workgroup and committee projects, as assigned. Assists other ACHC team members related to preventative & chronic care services and social determinants of health.
- Other duties as assigned.
- High School Diploma or GED required.
- Possesses a current, valid certification as a Medical Assistant, Health Assistant or LPN in the State of Iowa or obtains the certification within 12 months of employment.
- Bilingual is a plus!
