Responsibilities- Ensure that the individual service plan is followed; administer or supervise treatments prescribed by physicians.
- Make meaningful rounds to all residents, utilize service plans, and record visits on charts.
- Administer intramuscular and subcutaneous injections; perform in-house testing procedures; take and record vital signs.
- Document acute episodes or significant changes in resident status.
- Collaborate with Activity Director, DON, and Nurse Aides in carrying out the activities program.
- Receive or place calls to physicians and follow through with physician’s orders.
- Handle preparation and administration of prescribed medications; order and refills; chart administration as required.
- Complete charts and documentation in accordance with facility policy and state guidelines; supervise nutritional and fluid intake as directed; report persistent issues to appropriate personnel.
- Prepare incident reports timely and accurately; complete admission assessments and discharge documentation as needed.
- Supervise residents who self-administer medication following community and regulatory guidelines.
- Provide assistance with activities of daily living and offer emotional and social support to residents.
- Inform supervisors of any resident issues; respect confidentiality and maintain a professional demeanor.
- Be familiar with emergency equipment and procedures; maintain a safe working environment.
- Adhere to all policies and procedures of the Community; perform other duties as assigned.
- High School Diploma or GED; or one to three months related experience and/or training; or equivalent combination of education and experience.
- Requires Iowa LVN (LPN) license.
- Knowledge of general nursing theory and practice, infection control, universal precautions and OSHA standards.
- Familiarity with medical diagnoses, procedures, and administration of medications; understanding of potential side effects.
- Ability to lift at least 51 lbs and assist residents as needed.
- Proficient computer skills for relevant equipment and software; ability to read, write, speak, and understand English.
- Experience with memory care residents may be required for assigned areas.
