PCCR Community Inclusion Worker Position

Make a difference as a Community Inclusion Worker with Pacific Coast Community Resources and empower people to achieve their personal and community goals. Assist them in accessing vocational and volunteer opportunities.

Your role will entail providing personalized support and advocating for individuals with disabilities. This includes fostering their independence, facilitating community engagement, and creating meaningful relationships. You'll also collaborate with various stakeholders to implement comprehensive care plans tailored to the individual’s needs.

Key Responsibilities:

• Advocate for the rights of the individuals supported

• Facilitate and implement community participation activities

• Foster strong and supportive relationships within the community

• Participate in the development of Individual Service Plans

• Track daily activities and health documentation

Requirements:

• Grade 12 or equivalent educational background

• Community Support Worker Diploma or related field

• Minimum of 2 years’ experience in social services

• Valid BC Driver’s License and favourable driver’s abstract

• Criminal record check for vulnerable populations
