ABOUT THE PROGRAM:
Hendee Road House is a 6 bed residential program for men with severe mental illness and are discharging from long term hospitalization. The program will address the full range of daily living needs of each individual with a focus on increasing independence and self-efficacy of individuals through 24-hour/7-day a week staff support in the residence and the community.
ABOUT THE POSITION:
Case Manager
Duties & Responsibilities
- Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms.
- Maintains family and other collateral contacts to ensure quality, client-centered services.
- Assesses and involves natural supports in treatment planning and discharge processes whenever possible.
- Provides case management, clinical and crisis intervention supports as indicated.
- Demonstrates competency in age related or specialty issues and developmental needs for each population served.
- Uses EMR to document all services and contacts with clients, collateral contacts and appointments.
QUALIFICATIONS:
Education: Bachelor’s degree in human services required.
Experience: At least two (2) years experience in a mental health setting preferred.
Licensure/Certification/Registration: Valid driver’s license.
