Overview

Do you have the passion to make a difference to people’s lives following a neurological event? The Community Integrated Neuro and Stroke Rehab Service (CINSS) is a community‑based interdisciplinary therapy team supporting the timely discharge of patients from hospital. Patients have ongoing rehabilitation or neuro‑management needs following new or worsening presentations of neurological conditions, both stroke and non‑stroke.

The team works collaboratively with acute services, rehab and community networks and third‑sector services to deliver a patient‑centred, goal‑focused approach to rehabilitation. It is an integral part of the stroke pathway, reaching into both stroke and in‑patient rehab settings.

Responsibilities
  • Provide evidenced‑based specialist therapy assessments and appropriate intervention to patients referred to the service.
  • Follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as directed by line management.
  • Liaise closely with members of the interdisciplinary team and wider multidisciplinary team as required, facilitating effective caseload management.
  • Utilise specialist skill and knowledge to manage a neurological caseload.
  • Have general knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls, understanding the impact of multi‑pathological presentation.
  • Utilise a comprehensive range of evidenced‑based specialist assessments and treatment for patients in their own home or in a clinic environment.
  • Assist in the quality and activity monitoring systems, supporting evaluation of the effectiveness of the service.
  • Assist in the maintenance and development of the service.
Qualifications
  • Registered with the Health and Care Professions Council (HCPC).
  • Evidence of post‑graduate experience and sound clinical reasoning; commitment to providing high standards of care.
  • Evidence of immunisation and vaccination report from current UK occupational health provider or UK GP.
  • A full UK driving licence; international driving licence not accepted.
Additional Information

We are dynamic, friendly and committed to continuous development of clinical skills through joint working and an in‑house training programme. Successful applicants may be involved in quality improvement projects and service developments. The service is currently trialling a new Integrated Neighbourhood Team (INT) model; future line management may change but any formal changes will be preceded by appropriate consultation.
