Community Respiratory Diagnostics Nurse

This is an exciting and unique opportunity to deliver diagnostic spirometry to the clients of Change Grow Live (CGL) Coventry, the commissioned drug and alcohol treatment provider. The successful post holder will conduct clinics in CGL Coventry to diagnose respiratory conditions and advise accordingly on their management. The innovative project is paving the way for tackling health inequalities and bringing care to the most vulnerable within society.

This trailblazing project, funded by the Integrated Care Board, enables us to deliver care to those who typically do not engage with primary or secondary care, aiming to increase early diagnosis and reduce drug related deaths. The post holder will integrate across primary and secondary care. Support to be successful in this role will be provided by clinicians at CGL and the Community COPD Service and Respiratory Physiology department at University Hospitals Coventry and Warwickshire.

The post-holder will need to provide evidence of a valid Full UK driving licence and must have access to their own vehicle for work purposes.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

The post holder will be required to work closely with other clinicians and healthcare professionals, providing clinical advice, assessment and support for patients with respiratory conditions.

The post duration is 1 year and 10 weeks

responsibilities
  • Accountable for own actions in accordance with The Code of Professional Standards of Practice and Behaviour. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
  • Effectively communicate information regarding clinical decisions, policy and care pathways to patients/carers and the multi-professional team.
  • Effectively communicate sensitive information to individuals from all backgrounds who are experiencing health inequalities and may be considered as vulnerable. The post holder will be working with patients who are receiving treatment from the locally commissioned Substance Misuse Service and will need to communicate serious diagnoses which will require significant patient engagement and lifestyle change to improve their outcomes.
  • A skilled practitioner and clinical leader providing support to healthcare professionals and Service Leads to ensure service objectives are met, participating in COPD Service / Policy development and involving patients and public.
