An exciting opportunity has arisen to join the Neonatal Community Outreach Team as a Band 6 Neonatal Outreach Nurse. The successful candidate will work within the Neonatal Outreach Service, hosted by UHCW. This model of Neonatal Community Care is provided across the Coventry and Warwickshire region, including University Hospital Coventry and Warwickshire, George Eliot and South Warwickshire Foundation Trust.
The post holder will be an excellent team worker and will be responsible for ensuring the highest standard of clinical care and evidence based practice for infants within the scope of neonatal outreach, providing specialist on-going care for the neonate and advice, support and education to the family in the home environment.
The post holder will assist neonatal staff across the Local Maternity and Neonatal System (LMNS) at 3 sites in Coventry & Warwickshire (University Hospitals Coventry & Warwickshire NHS Trust (UHCW), George Eliot Hospital NHS Trust (GEH) and South Warwickshire NHS Foundation Trust (SWFT) with early and appropriate discharge planning in accordance with the criteria for neonatal community outreach care.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
- Support the development, implementation and evaluation of the Neonatal Community Outreach Service.
- Ensure a high quality, efficient and effective service, in support of an integrated LMNS approach.
- To work within the hub and spoke model for the outreach team.
- To carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of complex care programmes, ensuring continuous provision of integrated care across the LMNS in Coventry and Warwickshire.
- To provide a seamless timely discharge from hospital to the home environment
- Collect and collate data as required to support the outreach model
- Review patient pathways and facilitate early discharge of patients
- The post holder will take a proactive role in the education and training of the multi-disciplinary teams, providing on-going supervision as an expert in relation to neonates who require on- going care needs within neonatal community outreach.
- Allocate, coordinate, monitor and assess outreach service workload, ensuring resources are deployed appropriately, in the absence of the Clinical Nurse Specialist.
- Recognise and act as an advocate for patient, carer, service and organisation.
- Secondment or rotational opportunities possible for internal candidates upon agreeing with line manager prior to applying for the post
