Dorset Health Care University NHS Foundation Trust is recruiting Community Rehabilitation Assistants to support patients in their own homes, preventing hospital admissions and aiding early discharges. The role works within a multidisciplinary team alongside therapists and nurses, requiring good communication and a caring approach.
Full-time 37.5 hours per week (part-time also available). A car is required for home visits and basic IT skills are advantageous.
