Change Grow Live is a charity committed to making a real difference in the lives of the people we support. Our core values – Be open, be compassionate, and be bold – guide everything we do. We believe in treating everyone as an individual, providing support with respect and care, and working together to create meaningful change.

We are looking for a Prision In-Reach Recovery Coordinator to join our team in Coventry. Your role will focus on identifying, engaging and supporting service users with drug and/or alcohol needs, delivering targeted interventions and robust release planning to reduce substance misuse, re-offending and associated harm. You will work closely with prisons, probation services, and community-based providers to ensure seamless transitions and timely access to appropriate support on release.

A fantastic oportunity has arisen to become a Prison In-Reach Coordinator on a permanent basis, engaging service users who are engaged in a cycle of reoffending. This role involves weekly travel to our feeder prison, HMP Hewell, therefore access to a car and valid clean driving licence is essential for this role.

  • Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

  • Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
  • Deliver and promote Naloxone provision pre- and post-release, and train service users and peers in its use where appropriate.
  • Offer “through-the-gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
  • Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high-risk individuals and those at risk of disengagement.
  • Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high-risk service users and promote recovery and rehabilitation.
