Role DescriptionThe Care Coordinator is a full-time, on-site role based in Birmingham, responsible for organizing and overseeing care services for clients. Day-to-day tasks include assessing client needs, developing and updating care plans, and coordinating home care visits with support staff. The role involves regular communication with clients, families, and healthcare professionals to ensure care is delivered safely, effectively, and in line with individual preferences.
The Care Coordinator will manage case records, monitor service quality, respond to client queries and concerns, and help resolve issues promptly. This position also requires collaborating with the wider team to improve processes, maintain compliance with relevant regulations, and support positive outcomes for clients.
- Candidates should possess skills in Care Coordination and Case Management to plan, organize, and monitor care services effectively.
- Candidates should possess strong Communication and Customer Service skills to interact professionally with clients, families, and colleagues.
- Candidates should possess knowledge of Home Care practices to support safe, person-centered services in client homes.
- Relevant experience in social care, healthcare, or community support settings is beneficial.
- A relevant qualification in health and social care (or equivalent experience) is preferred, along with familiarity with care regulations and safeguarding.
- Ability to work collaboratively, manage time effectively, maintain accurate records, and use basic computer systems is required.
