Hawkesbury Lodge represents an important investment in supporting people across Coventry and Warwickshire on their recovery journey. Developed in partnership with Coventry and Warwickshire Partnership NHS Trust, the service combines a 14-bed residential rehabilitation service with an integrated Care Hub, providing a welcoming and recovery-focused environment. Together, these services offer personalised support to people living with severe and enduring mental illness, helping them build confidence, develop greater independence, enhance their wellbeing, and take positive steps towards living successfully within their local communities.

How you will make a difference as a Care Hub Service Manager

As a Care Hub Service Manager I empower people affected by mental illness to rebuild confidence, strengthen independence, and reconnect with their communities. Through focused, recovery‑based support, I help individuals set meaningful goals, overcome barriers, and access the resources they need to move forward and create lasting positive change.

As Care Hub Service Manager, I am responsible for the operational leadership and delivery of the Hawkesbury Lodge Care Hub, a structured day service providing therapeutic, psychosocial and recovery-focused interventions for residents and individuals from the wider community. I work under the leadership of the Head of Service (Registered Manager) and alongside the Residential Service Manager to deliver a fully integrated rehabilitation model.

I ensure the Care Hub delivers a high-quality, programme-based offer that supports individuals to build skills, engage in meaningful activity and progress towards greater independence. I work in close partnership with Coventry and Warwickshire Partnership NHS Trust (CWPT) and the Community Enablement Team to co-design and deliver an integrated model, including MDT working, shared care planning and aligned risk and escalation processes.

Key Responsibilities:

• Lead the delivery of a structured programme of therapeutic, psychosocial and recovery-focused interventions within the Care Hub in partnership with Enablement colleagues.
• Ensure the Care Hub operates effectively Monday to Friday, delivering a consistent and high-quality programme for both residents and community participants.
• Design, implement and continuously develop the Care Hub offer to meet the needs of the service specification and evolving population.
• Ensure all interventions are aligned to individuals’ care and recovery plans, promoting independence, life skills development and social inclusion.
• Support individuals to engage with structured activity programmes, including group work and 1:1 interventions.
• Contribute to achieving rehabilitation outcomes, including reduced dependency and successful move‑on.
• Work in partnership with CWPT and Enablement Teams to deliver integrated care, including participation in MDT meetings and joint care planning.
• Co‑design and embed Care Hub delivery in line with clinical input and system requirements.
• Ensure alignment between Care Hub and residential services to provide a seamless and flexible model of support.
• Oversee referral, assessment and review processes, ensuring timely access to the service and effective progression planning.
• Support the delivery of rehabilitation outcomes, enabling individuals to develop independence through supporting their goals and aspirations, including maximising their opportunities in community‑based living.
• Ensure appropriate management of risk within the Care Hub, including dynamic risk assessment and monitoring.
