An excellent opportunity has arisen for an enthusiastic ANP to join our friendly forward-thinking team to complement our existing clinical team at Kensington Road GP Practice.
Main duties of the job- As an Advanced Nurse Practitioner (ANP) you will be an experienced clinician who, acting within your professional boundaries, will provide same day care for patients registered at the surgery.
- This will include telephone triage, history taking, clinical assessment, diagnosis, treatment and evaluation of care, including assessment and diagnostic skills.
- We envisage the ANP will develop close long-term relationships with patients and work in partnership with them to achieve optimum health.

Clinical Care
Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
Undertake consultations with patients as an autonomous practitioner using own clinical judgement to diagnose, treat or refer patients
- Diagnose and manage both acute and chronic conditions, integrating both drug-and non-drug based treatment methods into a management plan
- Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
- Work with patients and their carers to support compliance with and adherence to prescribed treatments
- Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members
- Ensure patients receive high quality clinical care, delivered in a timely manner.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocol, within scope of practice.
- Ensure complete and accurate documentation of each and every patient contact.
- Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams to ensure patients needs are met.
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
- Professional
Adhere to the NMC Code of Professional Conduct and at all times work within the scope of professional practice.
Ensure that professional practice adheres to organisational and NMC policies, procedures and guidelines.
Maintain a professional manner and act as a positive role model to other members of the Primary Care team.
Maintain confidentiality with regard to information pertaining to patients and staff.
Deliver care according to NICE guidelines and evidence-based care
Communication
Communicate effectively with patients and families regarding complex information about their care.
Provide advice and information to patients, carers and their families where appropriate.
Communicate effectively and work collaboratively with medical, nursing, allied health professionals and operational staff to ensure delivery of a co-ordinated service.
Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers.
