Band 6 Children’s Community Nurse – CCN & SNSN
We are seeking an experienced and enthusiastic Children’s Nurse to join our Community Children’s Nursing Services. This Band 6 role combines working within the Special School Nursing Service (SNSN) during term time and the Children’s Community Nursing (CCN) Team during school holidays, supporting CYP with complex health needs across educational and community settings.
ResponsibilitiesDuring term time, you will provide specialist nursing care within special schools, supporting CYP with complex medical conditions, disabilities, and long‑term health needs. Key tasks include:
- Care planning and health assessments
- Medication management and enteral feeding
- Epilepsy management
- Staff training
- Contributing to Education, Health and Care Plans (EHCPs)
During school holidays, you will work within the CCN Team, delivering nursing care in homes and the community. Key tasks include:
- Support for CYP with complex and technology‑dependent needs, including tracheostomy care
- Continuing and palliative care
- Hospital discharge support
As a Band 6 nurse, you will provide clinical leadership, supervise and support colleagues, contribute to service development, and work collaboratively with families, schools, therapists, social care, and medical teams to deliver high‑quality, child‑centred care.
You will work autonomously, managing a caseload of children and young people, and will be involved in delivering clinical interventions, training parents, carers and staff from other agencies, and working with multi‑professional and multi‑agency colleagues to support children to achieve their best outcomes.
