Renal Post Transplant Clinic Sister

Responsible for looking after Renal post‑transplant patients, monitoring bloods, infection markers, medications, and wound care.

Responsibilities
  • Monitor patients' bloods, infection markers, medications, and wound status.
  • Coordinate staff and patients to ensure smooth clinic operations.
  • Run the clinic in the absence of the Manager.
  • Ensure mandatory training is up‑to‑date and PDRs are completed in a timely manner.
  • Manage patient telephone calls and e‑mail enquiries.
  • Transfer patients back to referring units.
  • Communicate clinical decisions, policy and care pathways to patients, carers and the multi‑professional team.
  • Contribute to service and policy development.
  • Provide clinical assessment and management for a group of patients, ensuring appropriate care is planned, implemented and evaluated.
  • Organise cross‑departmental activities including HR, resource and bleep holder responsibilities as appropriate.
  • Demonstrate technical skills for complex patient monitoring, equipment management and invasive procedures.
  • Develop and educate staff; participate in supervision, mentorship and appraisal.
  • Use IT systems to secure accurate patient, workforce and resource data.
  • Manage expected and unexpected clinical events.
  • Support patients and carers during difficult situations.
  • Promote Health and Safety and Trust policy, maintaining a clean and safe environment.
Qualifications
  • Current NMC registration.
  • Level 6 qualification (Degree/Diploma).
  • Evidence of continuous professional development.
  • Teaching and assessing qualification.
  • Advanced Renal course.
Experience
  • Nephrology experience.
  • Ability to successfully manage people and change.
  • Good communication skills.
  • Rotational experience within Nephrology.
Knowledge & Skills
  • Evidence‑based practice application.
  • Professional policies and procedures knowledge.
  • Clinical governance and commitment to clinical supervision and staff development.
  • Understanding of current NHS issues.
  • Policy/service development experience.
  • Renal disease and post‑transplant care knowledge.
  • IT skills.
  • Patient focused.
  • Excellent communication skills.
  • Prioritisation and deadline management.
  • Time management for self and others.
  • Decision making and problem analysis.
  • Venepuncture skill.
