Registered Nurse – Integrated Neighbourhood Team (Wound Healing Focus)
An exciting opportunity has arisen for an enthusiastic and motivated Registered Nurse to join the Integrated Neighbourhood Team (INT) at University Hospitals Coventry and Warwickshire NHS Trust.
Working as part of a multidisciplinary neighbourhood team, you will provide high-quality, evidence-based care to patients in their own homes, care homes and community clinic settings. You will be responsible for assessing, planning, delivering and evaluating care for patients with a range of acute, chronic and complex healthcare needs, promoting independence and supporting patients to remain safely within their own communities.
A significant element of this role will involve supporting patients with wound healing needs, including lower limb ulceration, pressure ulcers, surgical wounds, lymphovenous conditions and other complex wounds. You will undertake holistic assessments, develop evidence-based treatment plans and evaluate healing outcomes whilst working within agreed clinical pathways. Whilst wound healing will be a priority area of practice, the role extends to the wider management of patients on your caseload.
Nurses will be expected to deliver holistic care in line with individual patient needs, including medication management, insulin administration, long-term condition management, clinical monitoring and other community nursing interventions as required.

We work collaboratively across services to ensure patients receive seamless care that meets their physical, psychological and social needs.
As a Registered Nurse within the Neighbourhood Team, you will play a key role in delivering holistic care, coordinating treatment plans and supporting positive outcomes for patients across a diverse community caseload.
