Overview Outreach Worker – Multiple Disadvantage Team

Coventry | Permanent | Full Time

Trust may take time. Progress may begin with one conversation.

As an Outreach Worker within our Multiple Disadvantage Team, you’ll work across Coventry to connect with people experiencing rough sleeping, substance use and other complex challenges.

You’ll meet people where they are—on the street, in community settings and alongside partner services—building relationships that can open the door to accommodation, treatment, healthcare and a more settled future.

This is not a traditional nine-to-five role.

It is determined, compassionate outreach with real purpose.

The impact you’ll make

You’ll work with people who may have experienced repeated exclusion or found it difficult to engage with mainstream services.

Through consistent and assertive outreach, you’ll help people identify their priorities, understand their options and access coordinated support. You’ll work closely with housing, health, mental health, substance use and criminal justice partners to make sure plans translate into action.

You’ll:

  • Deliver street outreach across Coventry, including early mornings, late nights, weekends and severe weather.
  • Complete screening, assessments, recovery planning and onward referrals.
  • Build trust with people who may not yet be ready to engage with services.
  • Coordinate access to accommodation, healthcare, benefits and drug and alcohol treatment.
  • Provide harm-reduction advice and support people to explore positive change.
  • Work alongside partner agencies to deliver joined-up, strengths-based support.
Where you’ll work

Location: Coventry, working across street, community and partner-service settings.

Contract: Permanent.

Hours: Full time, 37.5 hours per week.

The outreach rota includes early morning, late-night and weekend shifts to ensure people can be reached outside standard service hours.

Please note:

Full-time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, the salary and any associated payments will be pro‑rata based on contracted hours.

This role is subject to an enhanced Disclosure and Barring Service check.

Responsibilities What you’ll bring

You understand that progress is not always immediate.

You’ll be persistent without being judgemental, able to hold professional boundaries and confident working independently in complex situations.

You’ll bring experience of supporting people with multiple unmet needs, which may include rough sleeping, substance use, mental health needs, contact with the criminal justice system or barriers to housing.

You’ll also have:

  • Strong communication, negotiation and advocacy skills.
  • Experience of lone working and managing risk safely.
  • The confidence to engage people who may not yet be ready to accept support.
  • Good organisation, prioritisation and record-keeping skills.
  • The ability to build effective partnerships across statutory and voluntary services.
  • A full driving licence and access to a vehicle.

Knowledge of Coventry’s local services, housing pathways and community networks would be valuable.
