The Triage Case Manager – RN is responsible for the assessment, treatment planning, intervention, monitoring, evaluation, and documentation of identified high-risk members. The role involves assessing and developing a care plan in collaboration with admitting, attending, consulting physicians, the member, and other healthcare practitioners. The goal is to effectively manage members on an outpatient basis to ensure appropriate level of care, prevent inpatient admissions and readmissions, and address members' medical, environmental, and psychosocial needs throughout the continuum of care.

• Current California RN License
• Triage experience preferred
• Acute Care or E.R. experience preferred
• Strong communication, critical thinking, and multitasking skills
