RN Outpatient Case Manager – Northridge – Working Nurse

Full Time, Covina, CA 91723

Pay Range: LVN $35.00 – $37.50, RN $45.00 – $50.00

• Assessment, treatment planning, intervention, monitoring, evaluation, and documentation of identified high-risk members.
• Develop care plan in collaboration with admitting, attending, consulting physicians, member, and other health care practitioners.
• Effectively manage members on an outpatient basis to assure appropriate level-of-care, prevent inpatient admission and readmissions, and meet medical, environmental, and psychosocial needs over the continuum of care.

• High school graduate required; minimum 1 year of experience and knowledge of Healthcare & Managed Care.
• Knowledge of computers, faxes, printers, and all other equipment.
• Proficient in MS Office programs (Word, Excel, Outlook).
• Typing 30 WPM with accuracy.
• Ability to handle confidential matters and work in a multi-task, high-stress environment.
• Flexible with changes in process and procedures.
• Critical thinking, experience with HMO, Medicare, and/or Medi-Cal.
• Good phone etiquette, professionalism, and strong analytical skills with attention to detail and accuracy.
