The primary function of the Home Health Registered Nurse is to demonstrate clinical expertise in nursing assessment, nursing diagnosis, and advanced proficiency in technical skills necessary for developing, providing, and coordinating care to patients.

Requirements include:

1-2 years in home health, meeting standards for care coordination and documentation, or

2 years of clinical experience outside of home health (e.g., Med Surg, ER, ICU). A Bachelor's degree is preferred; minimum of an Associate's degree.

JOB KNOWLEDGE/

SKILLS:

Knowledge of Medicare, Medicaid, and other funding source regulations as they apply to home health care. Demonstrate competence in advanced practice nursing skills, including the nursing process and interdisciplinary collaboration.

COMPUTER

SKILLS:

Familiarity with general computer functions, including user accounts, internet, email, file management, and navigation. Ability to manage online HR services and training programs.

TRANSPORTATION: Valid automobile insurance and reliable transportation are required.

ENVIRONMENTAL AND

WORKING CONDITIONS:

Extensive travel required, working in patients’ homes in various conditions, with safety considerations related to on-road travel.

Experience: 1+ to 2 years preferred.
