We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary care to homebound and palliative care patients in the Covington/Decatur/Lithonia area on a full‑time basis. The role involves applying medical techniques and principles under physician supervision and caring for a diverse set of patients with chronic diseases.
What You'll Do- Provide high‑quality care to primary and palliative patients in and around Atlanta County, GA.
- Assess patient health by performing physical examinations, obtaining and updating medical histories, and developing care plans.
- Effectively communicate with office staff and clinical operations across internal platforms.
- Deliver patient‑centered care to a panel of patients.
- Develop impressions, diagnoses, and treatment plans.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Prescribe medications, supplies, and other medically necessary interventions.
- Board certified with a valid NP or PA certification in Georgia.
- Strong clinical skills to accurately assess and diagnose patients.
- Positive demeanor and strong interpersonal skills.
- Ability to problem‑solve and operate independently.
- 1–2 years of primary care experience with geriatric patients; hospice experience highly preferred but not required.

$105,000 – $145,000+ (full‑time, based on experience and incentives).
