Nurse Practitioner - Covington

We are seeking an experienced Nurse Practitioner (NP) to provide high-quality, focused care in 1-2 nursing facilities. In this PM&R-centered role, you will round on patients, identifying and treating barriers to recovery such as mobility issues, pain management, wound care, functional decline, and rehabilitation needs. This position emphasizes efficiency, autonomy, and balance, allowing you to deliver targeted interventions without the demands of extended hours or emergencies.

• Conduct rounds in assigned nursing facilities, seeing patients twice per week to monitor progress and address recovery barriers in a PM&R framework.
• Manage an average of 5-15 patient encounters per day, with each interaction lasting approximately 5-7 minutes for focused assessments, treatments, and care planning.
• Collaborate with interdisciplinary teams (e.g., physicians, therapists, and facility staff) to develop and implement individualized recovery plans.
• Perform charting and documentation remotely from home using our efficient electronic health record (EHR) system.
• Maintain compliance with all regulatory standards, including those specific to PM&R and post-acute care.
• Decide and maintain your own consistent rounding schedule within daytime weekday hours, providing flexibility while ensuring patient needs are met.
