JOB SUMMARYThe Social Worker intervenes with patients who have complex psychosocial needs, requires assistance with eligibility determination for social programs and funding sources, and qualifies for community assistance from a variety of special funds and agencies. The role provides crisis intervention to patients and families with psychosocial needs and coordinates the development of a discharge plan of care for high‑risk patient populations.
Referrals for at‑risk individuals are received from interdisciplinary team members, including physicians, case managers, staff nurses, and other care team members.
- LMSW or higher licensure required and three or more years of hospital social work or protective services experience preferred.
- Working knowledge/experience in utilization management, managed care, and payer issues preferred.
- Experience in psychosocial and therapeutic counseling preferred.
- CCM, CMAC, or ACM preferred.
- Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity. Basic understanding of Medicare, Medicaid, and Managed Care regulations.
- Organized and efficient in establishing priorities and performing tasks within specific time frames.
- Self‑motivated requiring minimal supervision.
- Views problems as a challenge and investigates alternatives that demand creativity, flexibility, and the ability to work under stress.
- Responsible for maintaining strict confidentiality of all verbal & written information and documents.
- Familiarity with and appropriate use of community resources.
Must possess good physical health. Requirements include standing, sitting, or walking for long periods of time. Lifting at least 20 pounds is required.
