* JOB SUMMARY:

** The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies.  In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations.  

This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).

* PREFERRED QUALIFICATIONS:

*** LMSW or higher licensure required and three or more years hospital social work or protective services experience preferred
* Working knowledge/experience in utilization management, managed care, and payor issues preferred
* Experience in psychosocial and therapeutic counseling preferred
* CCM, CMAC, or ACM preferred
* Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.  Basic understanding of Medicare, Medicaid, and Managed Care regulations.
* Organized and efficient in establishing priorities and performing tasks within specific time frames.
* Self-motivated requiring minimal supervision.
* Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress.
* Responsible for maintaining strict confidentiality of all verbal & written information and documents.
* Familiarity with and appropriate use of community resources.
** PHYSICAL DEMANDS:
** Must possess good physical health.  Some requirements include but are not limited to standing, sitting or walking for long periods of time.  Lifting at least 20 pounds is required.
