SCOPE: Coordinates patient care requests—by phone and through Navigating Care or other future portal systems—for all providers—and serves as a resource to clinic staff. Responds to pharmacy calls and manages medication refill requests. Coordinates Provider orders, including home health, hospice, and hospital services. Serves as a liaison between physicians, patients, and external facilities. Provides back‑up support for clinic and intake staff.

Performs other duties as requested by physician/provider or supervisor.

• High School Diploma (CCMA), Nursing Diploma (LPN) required
• LPN licensure or CCMA certification required; CPR required
• 2 years experience required, Oncology experience preferred
