Job Description:

SCOPE:
Coordinates patient care requests-by phone and through Navigating Care or other future portal systems-for all providers-- and serves as a resource to clinic staff. Responds to pharmacy calls and manages medication refill requests. Coordinates Provider orders, including home health, hospice, and hospital services. Serves as a liaison between physicians, patients, and external facilities. Provides back-up support for clinic and intake staff.

Performs other duties as requested by physician/provider or supervisor.

ESSENTIAL FUNCTIONS:

* Triage patient calls

* Responds to Navigating Care Portal messages

* Perform Nursing Duties

* Coordinate patient care and documents

* Serves as resource for all staff

* Other duties as assigned

QUALIFICATIONS:

1. High School Diploma (CCMA), Nursing Diploma (LPN) required

2. LPN licensure or CCMA certification required; CPR required

3. 2 years experience required, Oncology experience preferred
