Community Inclusion Counselor at NEXUS

Support individuals in their daily life skills and growth with NEXUS Community Support Society as a full-time Community Inclusion Counselor. This role focuses on enhancing independence and quality of life through personalized support.

As a full-time Inclusion Counselor, you will assist individuals in achieving physical, vocational, and emotional independence. You'll work collaboratively with families and partners to develop tailored support plans that cater to their unique needs. Your contributions will significantly enhance community inclusion and positive life outcomes for those you support.

Key Responsibilities:

• Participate in assessment and program planning for individuals

• Develop and implement personalized support plans

• Provide person-centered support to promote independence

• Facilitate various recreational and educational activities

• Monitor progress and adjust supports as needed

Requirements:

• Certificate in Human Services, Social Services, or related field

• Minimum one year of related experience

• Strong communication and problem-solving skills

• Class 4/5 Driver’s License required

• Current First Aid/CPR certification is an asset
