Community Support Worker

Key Responsibilities:

• Develop and assess individualized support plans with clients and families

• Provide support for daily living and behavioral needs

• Facilitate social, educational, and recreational activities

• Monitor client progress and document accurately

• Collaborate with community agencies and stakeholders

Requirements:

• Certificate in Human Services or related field

• Minimum one year of experience in community support

• Strong communication and problem-solving skills

• Valid Class 4/5 Driver’s License required

• Current First Aid/CPR certification an asset
