Position Summary

The Hospice Nurse is responsible for the provision of professional nursing assessment, in accordance with a written plan of care, to maximize the comfort and health of patients residing in the home or in Long Term Care Facilities. The candidate recognizes and performs duties that need to be performed although not directly assigned. Duties fall within the scope and role as defined by appropriate regulatory agencies and governing bodies.

Key Responsibilities
  • Perform patient assessments, create care plans, and monitor patient progress.
  • Provide physical, emotional, and spiritual support to patients and families.
  • Administer medications and coordinate care with physicians, hospice staff, and caregivers.
  • Supervise hospice aides, providing training, guidance, and regular evaluations.
  • Maintain accurate and up-to-date patient records.
  • Communicate regularly with patients, families, and the healthcare team to ensure continuity of care.
  • Participate in care planning, discharge planning, and on-call duties as needed.
  • Ensure compliance with hospice policies, state/federal regulations, and infection control standards.
Education
  • Associate’s or Bachelor’s Degree in Nursing (preferred).
  • Technical school or college-level coursework in nursing is acceptable with relevant experience.
Experience
  • Minimum 2 years of professional nursing experience, including at least 1 year in an acute care setting.
  • Experience in hospice, case management, counseling, or end-of-life care (preferred).
Licenses/Certifications
  • Current and valid RN License in the state of employment (required).
Skills
  • Strong clinical assessment and care planning skills.
  • Ability to respond to the needs of patients and families in varied care settings.
  • Good organizational, communication, and teamwork skills.
  • Knowledge of hospice care, death and dying processes, and counseling (preferred).
Salary

$90,000.00/yr – $98,000.00/yr
