PACE RI is seeking a Dietitian to assess participants' nutritional status and plan disease-specific interventions as part of the Interdisciplinary Team. Provides direction to Dietary Aide, Food Service Manager and Day Center CNA staff involved in providing food service to participants.
The Dietitian will document in the EMR per CMS and PACE RI regulations, develop nutrition care plans for high-risk participants, and participate in care planning and program quality improvement activities.
