Main duties of the job
As an Advanced Nurse Practitioner (ANP) you will use your expert palliative care skills and highly experienced clinical judgement to contribute to the evidence‑based specialist treatment and care provided to patients with complex life‑limiting illnesses. You will be responsible for assessing, diagnosing and planning and implementing care to ensure optimum outcomes. You will also support the Community Cluster Team Leader with line management responsibilities and the daily clinical leadership of junior colleagues within the community team.
The role involves travel within the community setting, covering the Mid Devon area predominantly, and participating in seven‑day service provision, including on‑call duties.
- Provide expert specialist palliative care, applying advanced clinical judgement and knowledge to develop strategies for delivery of complex palliative and end‑of‑life care.
- Assess, diagnose, plan and implement evidence‑based care at the standard expected of an Advanced Nurse Practitioner to ensure optimum patient outcomes.
- Act as an independent and supplementary prescriber, participating in Non‑Medical Prescribing supervision to enhance care delivery.
- Demonstrate, teach and role‑model advanced communication skills and professionalism with patients, families, carers and other professionals to promote Hospiscare values.
- Manage, assess and review, in conjunction with the team leader, the daily clinical leadership of junior staff within the community team and deputise for the team leader in their absence.
- Design and deliver education and learning for all staff involved in palliative/end‑of‑life care, acting as a resource for wider professional and care teams.
- Suggest innovative ideas to improve service provision and forward the service in line with Hospiscare, Devon Clinical Commissioning Group and other government initiatives.
- Assist the Team Leader with line management responsibilities to promote good working relationships within the team.
- Attend, guide and contribute regularly to Gold Standards Framework (GSF)/Palliative Care and other multidisciplinary team meetings.
- Assist and contribute to Hospiscare’s clinical governance and audit process to support quality improvement.
- Assess and identify complex bereavement and communicate effectively with the Supportive Care team to enable seamless handover to Hospiscare’s Bereavement Care Services.
- Adhere to Hospiscare’s Infection Prevention and Control policy, ensuring a safe environment for patients, visitors and staff.
- Maintain a DBS Enhanced level check with Adults and Children Barred Lists.
- Advanced understanding of working with individuals at times of distress and conflict.
- Detailed understanding of key issues relating to End of Life Care, including national strategies and policy development.
- Advanced understanding of theoretical nursing models and their impact on care delivery.
- Awareness of change theory and its application in practice.
- Advanced skills as an expert practitioner, synthesising complex information using an evidence‑based approach.
- Advanced communication and interpersonal skills.
- Ability to manage challenging circumstances effectively and disseminate multifaceted information.
- Ability to prepare, lead and evaluate formal and informal teaching and education programmes.
- Ability to manage staffing resources effectively and efficiently.
- Ability to lead and work as part of a team, prioritise workload and manage time effectively.
- Ability to work autonomously, problem solve and act on own initiative.
