• Providing one‑on‑one rehabilitation services at Willow Falls in Crest Hill and Eden Vista Rock Run in Joliet, Illinois.

  Net Health

• Educate residents and caregivers on communication strategies and swallowing precautions.
• Complete accurate Net Health documentation and collaborate with the care team.
• Conduct one‑on‑one treatments with no group or concurrent therapy.
• Flexible scheduling and potential for additional hours.
• Reliable transportation and ability to travel between Crest Hill and Joliet.

• Active and current Illinois Speech‑Language Pathologist license.
• Master's or doctoral degree from an accredited Speech‑Language Pathology program.
• Certificate of Clinical Competence in Speech‑Language Pathology (CCC‑SLP) preferred.
• Geriatric, dysphagia, skilled nursing, or rehabilitation experience preferred.
