Position: Initiation Clinic Nurse

Employment Status: Full-Time (FT)

The Initiation Clinic Nurse plays a critical role in providing patient-centered care for individuals beginning Medication‑Assisted Treatment (MAT) for opioid or substance use disorders. This role conducts clinical assessments, coordinates initiation protocols, educates patients on MAT medications, monitors withdrawal symptoms, and supports safe, evidence‑based treatment in collaboration with the Medical Director (MD) and multidisciplinary care team.

This position screens eligible individuals for the Substance Treatment and Recovery (STAR) Program, coordinates closely with the MAT Coordinator for ongoing MAT services, and works with the Outpatient Detox Unit for clients who have higher acuity needs or require stabilization prior to MAT initiation. The Initiation Clinic Nurse also manages daily provider appointment coordination for initiation and administers all MAT medication injections for Crestview STAR clients.

• Strong assessment skills and ability to work with behavioral health and substance use populations.
• Motivation to support individuals in recovery.
• Clear and Active FL Licensed Practice Nurse (LPN) with ability to obtain an HIV/AIDS 501 Certification as needed. Specific experience in addiction medicine, behavioral health, primary care, or emergency medicine preferred.
• Bilingual/bicultural English and Spanish preferred (not required).
• Must be able to pass an Agency for Healthcare Administration credential test.
