Heartland Hospice is seeking an After Hours/On-Call Hospice RN to respond to patient needs after hours, including evenings, nights, weekends, and holidays. You will serve as the main liaison between patients, families, physicians, and the hospice team, ensuring timely, compassionate care and accurate documentation.
You will assess patient needs, participate in care planning, educate families, and collaborate with LPNs, CNAs, and other staff to maintain continuity of care.
